Canon 689

1. The pastor of the place where the baptism is celebrated,

must record carefully and without delay in the baptismal register, the names of those baptized, the minister, parents, sponsors, and also the witnesses, if there are any, the place and

date of the baptism, together with the place of birth and also

the Church sui iuris in which the baptized persons are to be

enrolled. 2. If it is a case of a child born of an unwed

mother, the name of the mother is to be indicated if her maternity is publicly established or if she requests it on her own

in writing or before two witnesses; likewise the name of the

father is to be indicated if his paternity is proven with some

public document or by his own declaration made before the pastor

and two witnesses; in other cases the name of the baptized is to

be recorded with no indication made of the name of the father or

parents. 3. If it is a case of an adopted child, the names of

the adoptive parents are recorded, and at least if it is done in

the civil records of the region, the names of the natural parents, according to the norms of 1 and 2 and with due regard for

particular law.
