Canon 761

A candidate for the order of diaconate or presbyterate, in order

to be licitly ordained, must give to the proper eparchial bishop

or major superior a signed declaration written in his own hand,

testifying that he is ready to receive sacred orders and that he

accepts the obligations attached to the same of his own accord

and freely and that he is to be perpetually devoted to the ecclesiastical ministry, at the same time requesting that he be admitted for the reception of sacred orders.
