Canon 1295

1. After the invocation of the Divine Name, the sentence must

express in sequence who is the judge or the tribunal; who is the

petitioner, the respondent, the procurator, with the names and

domiciles correctly indicated; the promoter of justice and the

defender of the bond, if they took part in the trial. 2. Next,

it must briefly report the facts together with the conclusions of

the parties and the formulation of the doubt. 3. Following

these points is the dispositive section of the sentence preceded

by the reasons on which it is based. 4. It is to close with an

indication of the day and place where it was rendered and with

the signature of the judge or, if it is a collegiate tribunal,

with the signatures of all the judges and the notary.
